    Does Your Insurance Plan Cover Services at the Wellness Corner?

    If you’re in an HMO plan

    Make sure your primary care physician (PCP) is in the St. Joseph Hoag Health network, which includes the following medical groups and affiliated physicians:
    • Hoag Medical Group
    • Hoag Affiliated Physicians
    • Mission Heritage Medical Group
    • Mission Hospital Affiliated Physicians
    • St. Joseph Heritage Medical Group
    • St. Joseph Hospital Affiliated Physicians
    • St. Jude Heritage Medical Group
    • St. Jude Affiliated Physicians

    If your PCP is not part of the St. Joseph Hoag Health network, you are permitted to change your PCP anytime throughout the year, and the new selection would be effective at the beginning of the following month. To assure your HMO benefits cover services provided at the Wellness Corner, you would need to select a PCP who is part of one of the networks listed above. Changing your PCP can be accomplished by following these steps:

    1. Call the Member Services number listed on your health insurance card.
    2. Provide the name of the primary care physician within the network you have selected, and specify you want to join his or her medical group’s name.
    3. Confirm your selection with your health insurance plan. Be sure to verify the medical group name matches the one you requested when you receive your new health plan identification card.

    If you’re part of a PPO

    You can change PCPs at any time. The Wellness Corner accepts most PPO plans to make it easy for you.
